The idea of the ALSTracker is that at some point during your illness you will have certain 'interventions'. You might take part in a clinical trial, take a drug (other than riluzole), or start taking a supplement. Many pwALS want to experiment. In this case, you may want to know whether or not something helps you to slow down the disease. This tool is designed to help you assess this.
What is Bayesian statistics, and why is it useful?Bayesian statistics is great at dealing with uncertainty. Instead of giving a single "best guess", it provides a range of possible outcomes and their likelihoods. This is really important when you want to make informed decisions.
Can this tool prove whether a therapy/supplement slows down the disease?No. The only way to do this is through well-designed clinical trials. However, this tool can give you an indication if something is working for you. Due to the non-linear nature of ALS, caution is advised when interpreting the ALSTracker report.
The idea is that you take measurements of your physical decline. Next, you define phases such as a reference phase with no intervention (other than riluzole) and a phase with some intervention (e.g., treatment, supplementation). ALSTracker estimates the probability distribution of the decline rate:
The common method is the ALSFRS-R questionnaire score (Here in german). But it requires quite some physical decline before it changes and does not allow you to follow the decline in a more fine-grained way over time. Depending on the type of ALS you have, there might be some other indicative measurements that may be used as a measure for your disease progression. Measurements you can use additionally to the ALSFRS-R score could be:
Before experimenting, quantify your decline rate / your measurement level without intervention. Maintain these measurements until the uncertainty about the decline rate / level is small enough, indicating high accuracy. This is the reference phase. Note that the delcine in ALS can be non-linear, so a meaningful reference period is crucial.
Phase comparisons
Once you have a solid reference phase, compare it with an intervention phase. Bayesian regressions will provide probability distributions of the relative difference between phases.

Simulated data
We also provide an example Excel file with simulated data that you can download and test the ALSTracker. We simulated data for the ALSFRS-R score, Vital Capacity (VC) and Neurofilaments (NfL) for one year. The measurement dates were made random between 20 and 80 days. The decline in ALSFRS score was 0.8 points / month during the reference phase and 0.4 points / month during phase "TreatmentA". The decline in VC score was 2% / month during the reference phase and 1% / month during phase 'TreatmentA'. The level of NfL was 50 pg/mL during reference and 35 pg/mL during phase 'TreatmentA':

ALSTracker was developed by Dr. Thorsten Wagner and Dr. Markus Stabrin. We are both scientists at the Max Planck Institute for Molecular Physiology in Dortmund, Germany. We both work in the field of structural biology, mainly in machine learning, software development and high performance computing, with backgrounds in computer science (Thorsten) and physics (Markus). Due to personal involvement we decided to develop this tool.
